报 名 须 知
1、参赛者须配合主办单位出席其安排的有关活动，包括但不限于采访、造型、培训、彩排、竞选、宣传、推广等。
2、已递交的参赛表格、照片、视频等不退还给参赛者。
3、参赛者同意主办单位可以：
1）在宣传、推广、寻找赞助商等与比赛有关的活动上，使用参赛者个人资料；
2）在任何地方制作及展示与比赛有关的节目时，使用参赛者个人资料；
3）为实现上述目的向第三方披露参赛者有关资料；
4）为实现上述目的而保存有关资料。
4、被选出进入总决赛之参赛者须与主办单位签署标准合约。
5、 如参赛者虚报资料或违反任何参赛规则，主办单位有权取消其参赛资格。
6、未满18周岁的参赛者须家长或监护人陪同并填写以下一栏。
7、主办方保留一切解释权。
Application Guidelines
1.All applicants must attend all functions organized by Evergreen Association, included but not limited to press conferences, various training, rehearsals, competitions, video taping, audio or other electronic forms for publicity, advertising, promoting and etc.
2.Photos and videos submitted by applicants cannot be returned after application forms are submitted.
3.All applicants agree that Evergreen Association is permitted to
1）Use applicants’ information for promotions, sponsorships and other activities related to the competition.
2）Use applicants’ information for the production and display programs related to the competition.
3）Disclose the applicants’ information to the third party for the above reasons.
4）Retain the applicants’ information and records for the above reasons.
4.All finalists must sign contact with the association.
5.All applicants cannot give false information and those who give false information will be disqualified.
6.Applicants, who are under the age of 18, must be accompanied by guardian/parents and fill out the form below.
7.In the event of a dispute of any kind, decisions of Evergreen Association are final.
